A 10-year-old patient presents to the office because of a one-day history of sore throat and fever. Two days ago his mother stated that he seemed more tired than usual and a "little off." He woke up yesterday with a severe sore throat making it hard to eat solid food. He has had a fever at home up to 38.9 C (102 F). He denies any sneezing, runny nose, or cough. He has some neck tenderness and has noticed some "swollen glands." He has no allergies to medications. Temperature is 38.6 C (101.5 F), pulse is 90 beats/minute, respiratory rate is 18 breaths/minute, and blood pressure is 90/56mmHg. Pulse oximetry on room air shows an oxygen saturation of 99%. Weight is 31.6 kg (69.5 lbs). Height is 134 cm (4 ft 4 in). Body mass index is 17.6 kg/m2. Physical examination shows an ill-appearing boy. Nose exam reveals no discharge. The oropharynx exam reveals enlarged, erythematous tonsils bilaterally with white exudates. Neck exam reveals bilateral enlarged and tender cervical lymphadenopathy. Remainder of physical exam is otherwise normal. A rapid strep test came back positive. What is the most appropriate medication to prescribe at this time?
