Question 

You are caring for a patient with a history of severe chronic pain who was admitted to your unit after a spinal surgery. The patient is using a Dilaudid PCA (patient-controlled analgesia) pump and has opiate PRN pain medications ordered including oxycodone and nurse-delivered PCA boluses. Your patient has been increasingly sleepy over the course of the day but continues to report "10/10" pain when you wake her up to ask so you've administered the maximum dose of opiates she is currently allowed. On her most recent VS check her respiratory rate is 7, she is difficult to arouse, and you had to increase her nasal cannula oxygen from 2 to 4 L/min to maintain oxygen saturations above 93%. Do you have clinical concerns? Explain your concerns and the actions you will take.
