Chief Complaint: "I am losing control of my bladder" History of Present Illness: Trudy is a 72-year-old retired preacher's wife who comes to your office and states that losing control of her bladder is embarrassing and that she can't go out "anywhere" because she needs to be close to a bathroom. She had this problem in the past, but never this badly. She denies burning pain with urination, hematuria, incomplete emp- tying, and urinary hesitancy. The problem is worse with coughing and sneezing. She comes to the office now because she is losing control of her urine on a daily basis instead of one or twice a week, and it seems more urine is leaking out than before. She does not have fever, chills, nausea, or vomiting; she does not have back pain or leg pain, leg numbness, or leg tingling. She had chickenpox as a child; she had her tonsils and adenoids out as well as her appendix before age 15. She had five children, all spontaneous vaginal deliveries with no complications. She was hospitalized for the birth of her children and when she had her gallbladder and uterus taken out. She takes medication to control heartburn and for.
