What are the differential diagnoses for a52-year-old Caucasian male with a history of permanent atrial fibrillation, CAD, dual-chamber pacemaker from St. Jude. The patient is alert and oriented. Reports he went to the bathroom to wash his face but felt weak, dizzy, and lost control, then fell sideways. He denies hitting his head against the floor, and he doesn't believe that he lost consciousness at any point. EMS sent him to ER. The patient states that he feels weak with poor appetite and has had an unintentional weight loss of about 25 lbs for the past five months. He states that he takes all of his medications regularly as prescribed. The patient states that he exercises by walking three times a day and riding a stationary bike five times a week. He denies any SOB, chest pain, palpitation, cough, congestion, fever, or chill at this time. MEDICATIONS: Aspirin 81 mg 1 tab PO daily, Amiodarone 200 mg 1 tab PO daily, Apixaban 2.5 mg 1 tab PO BID, Digoxin 125 mcg (0.125 mg) 1 tab PO daily ALLERGIES: NKDA. PMH: Permanent atrial fibrillation, coronary artery disease, H/O cardiac pacemaker, hx of diverticulitis. SURGICAL, SOCIAL, FAMILY HX: Cardiac pacemaker Smokes 1.
