A 22-year-old man presents to the office after recently being diagnosed with HIV. He reports a past history of intravenous drug use. He has no other past medical history. The infectious disease doctor he is now seeing for the management of his HIV is requesting that he be screened for tuberculosis. You opt to administer a purified-protein derivative skin test and have him return in 48 hours for evaluation of the test site. What would indicate a positive test in this individual?
