A 68-year-old man comes to the office with a three-day history of intermittent right forehead itching and throbbing sensation. He states, "I know I feel the sensation but there is nothing I can see on the skin. Doc, what is doing this?" He has not had such episodes before; it seems it is getting worse over the past three days. Physical examination shows increased sensitivity to light touch over an isolated patch of skin above the right eyebrow extending into the hairline but not into the temporal area.
