Question

AR is a 55 year old African-American male with a history of hypertension and diabetes. A few months ago, his prescription of sildenafil for erectile dysfunction was increased to 100 mg (50 mg wasn't effective). This seems to work well for him. Other medications include hydrochlorothiazide 25 mg daily; lisinopril 20 mg daily; and metformin 1000 mg two times daily. His blood pressure and blood sugars are well controlled; last HgbA1c was 6.2. AR went to the Emergency Department last night for new onset of chest tightness and shortness of breath. After thorough evaluation it was determined he was having stable angina, acute myocardial infarction was ruled out. He was sent home with a new prescription for sublingual nitroglycerin 0.4 mg prn for chest pain or tightness. He presents to you, his primary care provider, today for follow-up evaluation. What are your priorities for his care specifically related to medications?
