Pat is a 38-year-old man with a past medical history of HIV treated with dolutegravir/lamivudine (DTG/3TC). His last CD4 count was in the normal range, and his last HIV viral load was undetectable two months ago. He presents to clinic for persistent dysuria. He was treated for a urinary tract infection at an urgent care clinic one month ago; his urinalysis showed pyuria, but the reflex urine culture was negative. He was treated with cephalexin without improvement. He was seen in clinic one week ago for sexually transmitted infection testing, and three site gonorrhea and chlamydia testing were all negative (including urine).
