Question

A 33-year-old man with a long history of disabling depression with multiple suicide attempts is admitted. He has been married four times and was recently divorced again. He has one child who he is not allowed to see. He does relatively well when taking his medications but when he starts feeling "normal", he stops taking his medicine and eventually relapses. He is admitted on a 72 hour hold to your unit after threatening to shoot himself. Currently, he has not taken his medications for several weeks. He is quiet, making little eye contact, lying on bed facing the wall, and non-verbal except for short answers. Correctly, write your complete, priority nursing diagnosis for the client at this time.
