A 66-year-old man with 35 years of smoking history and family history of coronary heart disease take atorvastatin to reduce cholesterol levels and aspirin to reduce risk of coronary artery occlusion suddenly felt tightness in his chest which rapidly became so painful, and the pain radiate down his left arm. It was determined that the man was having anterior myocardial infarction. Since he has no relative contraindication to thrombolytic therapy including high blood pressure, stroke or recent surgery, the physician initiate treatment with both a thrombolytic agent, tissue-type plasminogen activator (tPA) and an anticoagulant heparin. Because of their low therapeutic index (TI), improper dosing of both drugs could have disastrous consequences such as hemorrhage and death, he was closely monitored, and the pharmacological effect of heparin was measured regularly by testing the partial thromboplastin time (PTT). The symptoms resolved in few hours and he was monitored for some days and discharged with the following medications' atorvastatin, aspirin, atenolol, lisinopril and clopidogrel for secondary prevention of myocardial infarction.
