Question

You will be providing care to a 66-year-old, English speaking female, admitted to the Medical Unit directly from her physician's office. She has a history of Congestive Heart Failure (CHF). Pt has 10-year history of hypertension, and was diagnosed with congestive heart failure 5 years ago. She also has Hyperlipidemia. She smoked 1 pack of cigarettes a day for 20 years, but quit smoking 2 years ago. Her immunizations are current with annual flu vaccine. Her history reveals a myocardial infarction 2 years ago. Since then, she has been on a low salt diet, lisinopril 10mg daily, Lasix 40mg daily, and carvedilol 3.125mg BID to treat her heart failure. She sometimes hides her Lasix from her daughter, who is her caretaker. She is now experiencing severe shortness of breath, cough, orthopnea and lower leg edema. She has an approximate 22-pound weight increase over the past 8 days. She appears thin and older than stated age. She is anxious, cooperative, and slightly confused. Her daughter is her caretaker and at the bedside.
