Case Presentation: 69-year-old patient Abigail has a history of cardiovascular disease. She was diagnosed and started treatment for hypertension 15 years ago. Since that time, she was diagnosed with hyperlipidemia and coronary artery disease. She takes two medications daily, lisinopril for hypertension and pravastatin for hyperlipidemia. Recently, she has felt increasingly exhausted, with shortness of breath and swelling of the legs. She's having difficulty getting up from a seated position and walking short distances. She has a hard time getting out of the car and walking into her home from the garage. Her primary care physician notes significant pitting edema of the lower extremities. A chest X-ray shows an enlarged heart with pleural effusions (fluid in lungs). An ECG was negative for atrial fibrillation. A cardiac echocardiogram estimated her ejection fraction at 38%.
