Vignette: Self-Determination You are an addiction professional who is working with a 24 year old woman, Melissa, with a long history of amphetamine abuse, who smokes meth daily. You have seen Melissa in your outpatient practice twice and therefore do not know her well, and were still working on building rapport at your sessions. What has been disclosed is that Melissa is homeless but usually is able to get a hotel or find other people to stay with, typically by way of prostitution or staying at drug houses. While she does have a history of suicidal thoughts and mental health treatment several years ago, she currently denies any intention to hurt herself or kill herself. She is ambivalent about stopping use of her drug of choice, meth, and reports that she is only attending sessions with you because it is a requirement of her probation. Today, you were contacted by a hospital physician (who does have a release of information from Melissa that allows you to speak with the physician) regarding Melissa. The physician discloses that Melissa has been diagnosed with treatable cancer 6 months ago. The cancer does have the ability to be lethal if it goes untreated.
