He admits he drank more alcohol than usual the weekend prior to the onset of symptoms. He works the night shift as a security guard. He denies recent travel, chest pain, orthopnea, paroxysmal dyspnea, and peripheral edema. On the physical exam he is awake, conversant, has an irregularly irregular pulse at 130-140 beats/min, and a BP of 150/100. He does not have proptosis, chemosis, abnormal EOMs, or thyroid enlargement. His lungs are clear, PMI is nondisplaced. There are no murmurs, gallop, rubs, or peripheral edema.
