Mrs. Hargrove. is a 65-year-old African American woman who was in her normal state of health until she presented at her nurse practitioner's (NP) office complaining of ''feeling cold and tired'' for about a week. Mrs. H.'s medical history contains several chronic medical conditions. All of her known medical conditions are currently well controlled. They are as follows: hypothyroidism (controlled with 175 mcg of levothyroxine sodium every day), diet-controlled diabetes mellitus (HgbA1C normal at 5.5%), hyperlipidemia (total cholesterol level 155 mg/dL), gastroesophageal reflux disease, and an overactive bladder. Mrs. H. has always been conscientious about keeping her healthcare appointments and is current with her gynecologic, ophthalmologic, and podiatric care. This patient was hospitalized for the surgical repair of a ventral hernia in 2001 and of renal calculi in 2003. Her last screening colonoscopy was in 1998, and the findings were within normal limits.
