Question

Differentiating the relationship between serum calcium and PTH levels can be a bit confusing. Understanding the underlying mechanisms for the alterations can assist in interpreting these laboratory values. The following are possible laboratory findings. Answer the following questions.

1. Explain the mechanism by which serum calcium and serum PTH are high with primary hyperparathyroidism caused by adenomas.

2. Explain the mechanism by which the serum calcium is high and the serum PTH is low with a non-parathyroid disorder such as breast malignancy.

3. Explain the mechanism by which the serum calcium is low and the serum PTH is high with secondary hyperparathyroidism.
