Mr rupinder patel is 73 year old reture business man. Rupinder has a medical history of heart failure with reduced ejection fraxtion. This developed after he experienced the two mycardial infraction 11 years ago. Both ventricles were affected. Previous shet radiology showed left ventricular hypertrophy. The death of wife 2 years ago has led to Rupinder experiencing several episode of depression that haa been exacerbated by his sons both moving to western Australia for work. This lonliness and sadness makes it difficult for hin to be concordant with his heart failure management abd susrain the necessary lifestyle adjustment required to prevent exacerbations, which result in several admissions to hospitals for management and review of his heart failure.For this current admissions, Rupinder was referred to hospital by his nirse practitionee after recently rapidly gaining weight. The time now is 0800 and you have just come on morning shift. Rupinder has been on the wards for only two hours after spending approximately 12 hours un emergency waiting for a bed to become available. Rupinder appears slightly disoriented and tells that he spent his night in recliner chair bedise the bed sitting upright because this is only way he can get his breath.
