Mr. Johnson is a 63 year old African American male with a history of Type 2 DM, HTN, and Heart failure with preserved EF (55%) who presents to establish care in your clinic. Mr. Johnson was previously seen by another provider group that was in the suburbs but hasn't had follow up since his wife died of Alzheimer's dementia 10 months ago. He had to take a leave from work and doesn't currently have his retirement package. He has been doing COBRA insurance. He wanted to start care at a new practice that wouldn't make him feel more comfortable. He also states, "I'm not a big fan of doctors."

1. Could there be "health system" or provider-related reasons why Mr. J might not be a "big fan of doctors?"
