The nurse is caring for Harold, 54 years old, who is admitted to a psychiatric daycare program for social skills training following discharge from a healthcare facility with a diagnosis of schizophrenia. The client reports that he has had problems with auditory hallucinations, which have caused him problems in the past. He also reports that he has not always taken his medications as ordered. Harold is dressed appropriately in worn clothing. He appears unshaven but clean. The client makes no eye contact and has a blunted affect. Harold answers questions appropriately. Harold states that it has been difficult for him to maintain a job or friendships, saying, "I just can't seem to keep my promises."
