Question

The case presented for this assignment provides an overview of the health status of a female patient named AD, who is 80 years old. She visited our office for a Chronic Disease Management follow-up. The patient was diagnosed with Type 2 diabetes mellitus without complication 15 years ago. Her A1C levels have fluctuated over time, with the latest result at 7.3 in January 2024. Additionally, she has been experiencing urine microalbuminuria since January 2021, which was still present in December 2022 but resolved in May 2023. Her last eye exam in November 2021 revealed no signs of retinopathy, but she will need a referral to a new office next year. The patient is currently taking Trulicity 0.75mg, Lantus 13 units, Humalog 9/9/9, SSI, and avoiding metformin due to her GFR of 30.
