Question

This client has diabetes and often feels numbness and tingling in their hands and feet due to diabetic neuropathy. This condition has resulted in a decrease in sensation and puts the client at risk for burns or injury. When the client feels pain in other areas of their body, they are able to verbalize this to staff. This client is able to use the bathroom independently and does not require the use of incontinence products. This client can also walk with the assistance of a walker, however they frequently become tired with minimal exercise and take frequent naps throughout the day. When laying in bed, sleeping, or napping; the client is able to change body positions unassisted. The client has a small appetite and frequently misses meals due to napping. When they eat, they usually only eat ½ of the food offered to them. When sitting at the dining table in the chair, the client is able to maintain good posture. Braden Score:
