Case: According to the American Counseling Association (ACA, 2014) Code of Ethics, assessment is "the process of collecting in-depth information about a person in order to develop a comprehensive plan that will guide the collaborative counseling and service provision process" (p. 20). The intake will have several different parts:

Introduction
Explain confidentiality and the exceptions
Reason for the referral
Current situation, for example, living conditions, work, school, etc
Current and past medical history
Current and past psychiatric treatment
Substance abuse history
Family History
Education and work history
Cultural and Spiritual influences
Strengths and weaknesses.
What the client wants to get out of therapy

Intake interviews should take at most 40 minutes to 1 hour. I would use motivational interviewing skills such as open-ended questions, reflective listening, and summarization to let the client know that I am listening and understanding what they are trying to convey.
