Question 

A pt comes in with jaundice there are a few questions the nurse can ask to determine the cause. Jaundice is a greenish-yellow discoloration of the skin, it is a sign of a disease but is not a unique disease it accompanies many diseases that may be indirectly or directly affecting the liver. The first thing the nurse would ask would be when the symptoms started and how long has it been present. The nurse would ask the patient if they have noticed a change in color of urine and stool. And the nurse would want to know if the patient is experiencing any other symptoms such as itching, abdominal pain or fatigue.
