Question: Human error causes a lot of occupational injuries and illnesses. Look at any number of the roughly 300 confined space emergencies that occur annually and lead to illnesses or injuries. There are so many factors and other safety disciplines that are a part of confined space. A single error in say ventilation of the confined space can result in an atmosphere now becoming toxic for the workers in the confined space who may or may not be proficient in using their air monitoring equipment. Ventilation is interrupted and say the meter is alarming for lel the meter is calibrated with pentane and atmosphere in the confined space used to have acetone stored before being shut down for maintenance.

Three (3) minor human errors with the ventilation, confined space attendant, and air metering group could easily cause an injury or illness. During our OSHA training at Cincinnati State, we really hammer home the human errors through case studies, real world examples, and safety videos. Especially for the new workers that have been hired by our clients and getting their HAZWOPER and confined space training, because we always see on the job site hey have the new kid do the work. Many young workers are lacking the real-life hands-on hazardous workers experience that their peers have and can lead to more frequent errors in the occupational setting.
