Question

A client is admitted to the locked unit because of frequent auditory hallucinations. Voices are telling the client to harm himself. The client is being given haloperidol IM. The nurse will include in the plan of care which primary intervention to provide a safe environment for the client? Assess alertness every 15 minutes. Monitor for neuroleptic malignant syndrome (NMS). Monitor for tardive dyskinesia (TD). Monitor intake and output every 2 hours.
