Question

Proofread this: Pamela is here to establish care. She has several concerns today including a change in a chronic insect bite on her left arm, a bump on her left forehead, and an increase in the frequency of dizziness/headaches. In regards to the insect bite on her arm, she previously has seen Dr. Peraza for evaluation and states the insect bite has become raised and itchy as of recently, The bump on the left side of her head has been present since her cranioplasty and states she is not having any pain. Her dizziness and headaches are more frequent, occurring almost every day she works. She states neuro recommended to cut back on work hours, however, states she typically works 5 days a week as a bartender. She takes Tylenol with good effect for her headache, and states the dizziness is usually following any bending required of her at work or standing up fast, and is usually relieved with rest.
