QUESTION 1
Richard, aged 16 and employed as helper by Wood World Limited, sustained injury to his left index and ring fingers when they came into contact with the moving blade of a circular saw machine which Richard was using to cut a wooden pallet of dimension 1000 *100*80 mm. Richard went to a nearby hospital for treatment where he was admitted. Richard was discharged from the hospital after 24 hours. Enquiry carried out at the governmental level revealed that:
(i) The blade of the said machine was not guarded to prevent access to it.
(ii) Richard was neither informed, instructed, supervised nor trained on the precautionary measures to be observed when working on the circular saw machine.
(iii) Management of Wood World Ltd admitted that Richard, being 16 years old, is a young person and should not have been authorized to work with wood working machines unless being trained and supervised to do so.
(iv) Management of Wood World Ltd had also not notified the authority concerned about the occurrence of the accident by sending the report of accident.
