Question 

A 75-year-old man is admitted to the cardiac care unit with anterior ST elevation myocardial infarction (MI) and undergoes cardiac catheterization with stent placement. He does well and is released home in good condition. However, he is brought back to the hospital 2 weeks later with crackles at the lung bases, grade 2+ pitting peripheral edema on his legs, and severe dyspnea at rest. Findings on electrocardiography indicate Q waves on anterior leads, but no new acute changes are seen. Cardiac enzymes are sent to the laboratory. On auscultation, you hear a holosystolic murmur 3/6 on all fields that was not present before. What complication is the most likely to be found using emergent echocardiography?
