Question

What is the diagnosis A three day old baby boy was admitted in Pediatric unit due to fever and foul smelling discharge and redness in the umbilical area as verbalized by mother. On the third day of admission, Nurse Sofia was assigned to the patient at 7-3 shift.The patient was cuddled by mother trying to calm him down from a loud and lusty cry. Mom attempts to breastfeed patient but was irritated due to poor milk production as she claimed though patient has a good sucking ability, so she gave milk formula to satisfy him. Upon initial assessment of Nurse Sofia at 8 in the morning, patient was hot to touch, with generalized jaundice, but no skin breakdown noted. However, the umbilical area was noted to have redness with foul smelling discharge. Vital signs checked as follows; Temperature 38.5 degrees celsius, RR- 62/bpm, HR- 130/bpm. With IVF of D5 IMB 500cc attached to infusion pump set at a rate of 15 mgtts/min at right hand. She checked the IV site and noted redness and swelling and patient cried when palpation on the site was done, no back flow noted as well. She gave paracetamol as prescribed at 8:05, and subsequently.
