Nursing Note: Mrs. Ida Adams is a 93-year-old female client in room 505D. She was admitted to the floor with altered mental status last evening. Her daughter found her watering the cement patio instead of the flowers, and she became agitated when questioned. Ida's daughter reports that her mom is usually fully alert and oriented. Ida has very minimal health history and only takes calcium, vitamins, and omeprazole daily. Ida's daughter reported that she had an infection the last time she became disoriented like this. 9/29 0810 Nursing Note: Client indicates 0/10 via nonverbal pain scale.
