Scenario: A worker was assigned the tasks of dismantling a metal scaffold inside a reactor. The worker was at the base of the reactor after dismantling the scaffold. The worker was required to climb up 12 m to exit via the overhead reactor manhole. The worker used a vertical rope ladder to climb up and while he was on his way up, he fell and landed at the base of the reactor. The worker was immediately rescued but he died on the way to the hospital.
