Question

It is April 12, 2024 you are a new primary care CRNP at a federally qualified health center (FQHC). XJ is a 68-year-old male who presents to your health center with multiple concerns. He is most concerned with new troubling symptoms that started about 8 weeks ago including frequent urination up to 15 times a day and 3-4 times a night, and "constant" thirst and hunger. He also reports a weight loss of about 6 pounds in the last month. Additionally, he has one week history of mild shortness of breath, productive cough and fever. At the end of the visit, XJ states that she needs his thyroid medication and inhaler refilled.
