Question

SW revied a call from PCG ( Kitty) requesting a PRN SW visit to dicuss patient care. I schedule a PRN for today.Upon arrival to the home John was present as well as pt grandaughter. The family was very tearful stating that the medication were not working and that the 98-year-old patint has been suffering and they felt as though hospice has not been providing the comfort she should be giving. THe family stated that they have ben doing some research and that they printed off information on terminal agitation which John was reading. John headed me the information and they asked what did SW know about this. I explained to the family that it is a form of decline when the pt is in the transitioning phase. SW ten turn to ask John jhas he consulted optum and contact Morgan to make her aware of the concerns. John replaied no he was going to read the information the family printed off first then called optum and Morgan when he got back to the office.
