Case Study

A Jeremy is a 70-year-old man who is beginning to struggle with the onset of dementia, and this is affecting his ability to live independently, in a safe manner. He currently lives by himself, in a ground floor flat near the city of Melbourne. An initial assessment has stated that he needs to move into a care environment, due to dangers in his current living environment. Jeremy enjoys the freedom of his independence and is anxious about moving into a care home. He is especially worried that a new environment will be completely unfamiliar to him, and that this will worsen his condition. Other than struggling with the early stages of dementia, Jeremy has no other health problems that need to be considered. You have been asked to facilitate Jeremy's move into a local care home, which will include making sure that his needs for a stable and familiar environment are met.
