a. How much of a connection to yourself (your soul or spirit) do you feel you have and how are you searching for meaning and a sense of purpose in your life (describe your feeling of connection or lack of connection as well as your search for meaning and purpose)?

b. What values and beliefs are most important to you and do you act compassionately in accordance with those tenets (please be descriptive and specific)?

c. Do you think that you are present and mindful in your everyday life or do you struggle with distractions? How so or how not?

d. What concerns do you have about barriers that may stand in your way of spiritual well-being -- and what thoughts do you have about overcoming them?

For question A you can write depending on this. Hint about myself. Religion is a significant aspect of my life because it provides hope and enhances inner peace in my life. It facilitates my capabilities to cope with different challenges in life. I am an orthodox Christian who frequently seeks solace in my faith, participating in religious practices like attending church.
