Question: You will complete this project in several stages. You will be completing a Quality Improvement plan and implementing one intervention. There are two documents you will submit for this assignment.

Step #1: Problem statement. Discuss with your preceptor and faculty a problem on your unit. Look for articles that support the need for change. Write your problem statement and answer questions on the Quality Improvement Plan document.

Step #2: Set an AIM using the Model for Improvement. Follow directions on the Quality Improvement Plan document.

Step #3: Explore and identify change ideas (solutions). Identify two solutions and choose one for your first PDSA cycle. Answer questions on the Quality Improvement Plan document.

Mid-point: During your scheduled midpoint meeting with your faculty, discuss your QI plan.

Step #4: Complete one PDSA cycle and answer the questions on your PDSA and Results document.

Step #5: Design a chart showing your results and place on the PDSA and Results document.

Step #6: Write a one-page scholarly reflection providing a brief outline of your project and describing the results of your QI project. Did you see an improvement? If not, what would you do next? What did you learn in this process? Any barriers, successes and lessons learned?
