Question: Choose a health-related behavior you would like to increase or decrease.

Use the SMART goals framework to set a behavior change goal for your target behavior

Monitor and record your behavior for one week (Week 1) without making any changes to your typical routine. This will be your baseline data (i.e., your pattern of behavior without any intervention.)

Choose and implement ONE strategy to change your behavior to achieve your goal. This should be based on theories and concepts we learned in class. Apply this strategy for one week on Week 2.

Evaluate the success of your strategy.

If you were unsuccessful, modify the strategy or try a new strategy on Week 3. Continue monitoring and recording your behavior on Week 3. Be sure to record any necessary additional information that will illustrate if your new strategy was effective.
If you were successful, evaluate any weaknesses in your strategy (as in the example above). Modify the strategy or try a new strategy to improve your situation.
Repeat for Week 4 (i.e., Evaluate Week 3's strategy. Modify the strategy or try a new strategy.
Continue monitoring and recording your behavior on Week 4.)

Describe your baseline data and your thoughts or observations about your baseline behavior.

Explain strategy 1, how you measured your behavior, and your evaluation of strategy 1.

Explain strategy 2, how you measured your behavior, and your evaluation of strategy 2.

Explain strategy 3, how you measured your behavior, and your evaluation of strategy 3.

Reflect on your experiences over the last 4 weeks. Were you able to achieve your goal? What did you learn?

Thoroughly explain how your experience connects with the theories of health behaviors and behavior change we learned in class. Choose at least one relevant theory, describe that theory to demonstrate your understanding of it, and clearly explain how it is relevant to your
