Explain why these key elements and health indicators are useful, My health issue is about the increasing incidence rate of breast cancer in Westchester County. For my community health assessment and healthy community assessment, the key elements I would do are the following: Education - Are patients educated about getting screened for breast cancer and the consequences of not getting checked? If not, what education can we provide at the medical center that is easy to read so they understand? Income/Insurance - Does the patient have insurance? If not, are they eligible to apply for insurance? If not, are they enrolled in the state's Cancer Services Program to cover screening costs? Family History - Do they have a family history of breast cancer? If so, are they seeing their medical provider? Are they seeing a genetics specialist? Geography - Where does the patient live? Are they in an area where they cannot get annual mammogram screening because it is unsafe to go out? Transportation - How are they getting to the doctor's office? Bus, train, walk, carpool?
