Question 1: June is a 25 year old elementary school teacher. Several months ago, while watching her students in the playground, she became extremely anxious; she felt her heart pounding and thought she might faint. Although she recovered within 20 minutes, she experienced these same symptoms a few weeks later in the school cafeteria and not long after, while teaching her class. June has now become apprehensive and is fearful about going to work each day. She has been taking so many sick days because of this fear that her job is now in jeopardy.

Question 2: June's life over the past year has been difficult. Her mother died, and she has been caring for her father with whom she lives. She has tried to maintain an active social life but has found that difficult because of work and family demands. In addition, she has been diagnosed with endometriosis. Before this year, she functioned well, had several friends, and was involved in many activities. She had received high ratings at work.
