Identify and explain two formal ways to gain patient feedback and two informal ways of gaining feedback and how you plan to  incorporate this feedback into your work to improve the quality of care being provided.

  • For formal feedback mechanisms consider such things as HCAHPs (hospitals), CAHPS (long term care), HHCAPS, (home health), Press Ganey, etc.
  • For informal feedback consider patient complaints, leadership rounding, etc.
