Take the following into consideration. Paroxetine (Paxil) can increase blood glucose levels in diabetic clients. A solution would be to let her PCP or endocrinologist know she is on Paxil and maybe have the provider increase her diabetic medication. Since Paxil has been working to manage her depression we can work to try to address the side effects. So that is one of her complaints, the increased blood glucose.

The patient's weight gain is another complaint. That is also a known side effect of SSRI, we can provide education regarding the benefit of physical exercise to see if that would be sufficient to address the weight gain. We can try an exercise regimen that works for the patient and see if that resolves the weight gain issues as one alternative

We have to taper the paroxetine (paxil) and maybe start at low dosages citalopram or escitalopram. For citalopram we have to get an EKG prior to starting the medication due to potential of increase QT interval.
