Assignment:
The patient was a female patient, age 25, who was diagnosed with major depression disorder and schizoaffective disorder, which caused her to have suicide thoughts. Moreover, the patient had Tourette syndrome. Patient was well-nourished, calm, awake, and comfortable, but she was having trouble communicating. She kept forgetting what she was going to say in the middle of sentences. I assessed the patient by questioning her about her sleep quality, to which she responded that it was good, whether she had any suicidal thoughts or not, which she denied, and what coping techniques she used to deal with her sadness.
The patient said, "Deep breathing, colors for bad and good feelings." Medications like ARIprazole 10 mg tab PO nightly indication for mood stabilization, guanFACINE 1mg tab PO BID indication for ADHD, Lithium 600mg cap PO daily indication for mood, Melatonin 5mg tab PO nightly indication for sleep, and OLANZapine 15mg tab PO nightly indication for mood were prescribed. After that me and my fellow classmates had a conference with our clinical professor, and we discussed our experiences. Our prof asked us questions and we answered. At the end of the conference our professor answered the questions we had and explained the procedures we need about mental health patients.
What care plan would you choose for this patient include assessment, diagnosis, outcomes and planning, implementation, and evaluation?
