It's midday at City Center Hospital, and your next draw is for Thomas Phelps in room 322. As you enter room 322, you see the patient sitting up in bed. You greet him. You ask his name, and he replies: "I'm Tommy Phelps." You then check the ID band on his wrist-the number matches the number on the requisition form. When you ask him to state his date of birth, he says, "June 14, 1985"-but the requisition gives the date of birth as July 14, 1985. Can you go ahead and draw? Please explain the potential consequences.  What would you do to correct this?
