Mrs. Q. is a 35-year-old African American woman you have been seeing at the clinic and treating for schizophrenia. Her current medication regimen includes: Risperdal (Risperidone) 3 mg QHS daily, and Zoloft (sertraline) 50mg, one every morning. You note that she has gained 10 lbs. since her last visit three months ago and a total of 25 lbs. since starting Risperdal.

Today she comes to the clinic for a medication check and reports that overall she is doing well. She believes the voices are quiet enough and says, 'I can do the things I need to do without being bothered like I used to be.' Now she reports that she has noticed some stiffness in her right arm. She adds, 'I feel like my legs have somewhere to go but I don't have anything to do.' Finally, she asks, 'Can you tell me why my breasts have been leaking milk lately? I know I'm not pregnant.' Her youngest child is four and was weaned two years back. An AIMS (Assessment of Involuntary Movements) was completed and a stiffening of the right arm was noted. You are also aware Mrs. Q. has a family history of diabetes (father and sister).
