CASE STUDY 1: CONSTRUCTION SITES ARE A "DEATH TRAP" The City of Tshwane appointed a construction company (Khato Civils) to install bulk water, sewer services, and internal water reticulation in the shack settlement. However, a young boy drowned after falling into a trench that was left open by the construction company. In addition, it's alleged that the incident was not the first where a person died involving the construction company. However, the Company Chief Executive Officer (CEO) disputed the residents' version that the trenches were covered only on the day Tshiamo died. The CEO said that all manholes and open trenches were closed. The company's preliminary investigations revealed that the construction manager failed to secure the safety of the site, despite having been instructed to do so. As a result, the company suspended the site construction manager and charged him with gross negligence and gross dishonesty.
