Case study: Rhonda is a 42-year-old woman who lives alone. She does not like the environment in which she lives, it is very congested and has nowhere to go. She had diabetes for 10 years and is overweight. Doctors say if she doesn't lose her weight, she must go on insulin soon. Doctors put her to meager calories of 500 a day with shakes, vitamins, and celery, but her sugar was all over the place within the first few days, so she had to stop eating healthy. She must have stuffed herself with some things she does not remember, but she had junk food in the fridge along with her healthy food. Her diabetic nurse said she needed to exercise 30 minutes daily. But she is scared. She is up early to go to work and doesn't get home until after dark. And there is nowhere safe to go walking around in her place, where she is living, and there are no walking groups. And when it's dark, she can't do it. She has no friends other than the girls at work, and that's only while she is at work, not on weekends. She doesn't like the way she is she hates it. People can be so cruel because they stare and say hateful things.
She said she is not a bad person being emotional. She said she knows about healthy eating and mentioned her mother being diabetic and her dad being abusive. If her father caught them eating, he would go off. I had a habit of eating when she was scared. Growing up with her family was stressful. She hid her eating when she was younger. She used to run and take laxatives to remain thin, and her father never knew about her eating. She still eats whenever she is anxious or sad. She mentioned she needed help with her anxiousness and sadness, not counting calories or the number of slices of BREAD she eats. When her mom died, she was 22 years old, and her father left her without saying anything. There was just her. She stopped taking laxatives but kept eating. She knows what she needs to do to be healthy, but there's only one store nearby where she can get decent fresh food; she can only go there on Saturday if she is not working other than she takes junk food every day. She narrated all the above things, being very emotional.
Question: The scenario you need to consider to complete these assessments, is that you are a third-year nursing student and you have been placed in the Community Health Setting where Rhonda lives. This may be the local Community Health Centre. As part of your placement, you are to conduct an education session. This session may be an individual education session (one on one) with Rhonda, or it may be a group session (with clients with similar backgrounds and physical conditions as Rhonda). You can choose which you prefer/or which might be more suitable for Rhonda. You must first identify and then justify the need for such a session with your evidence (Assessment 2) and then conduct this session (Assessment 3). See the assessment briefs for the detail. You will need to really consider the context for Rhonda: • Where does Rhonda live?
• What is her ethnic or cultural background?
• What is her age?
• What are her presenting physical conditions?
• What are the impacts of these physical conditions on her lifestyle and wellbeing?
• What is her story?
• What is her living environment or arrangement like?
• Does she work? The nature of her work?
• Her income or financial support? And so on .... Some of this information will be available from the video but not all. Think holistically, think about the value of working with community and working with people in community settings, think about issues of health equity and access, community engagement and participation, as well as capacity building and advocacy.
