Scenario: Setting the scene
Refer to Case Study 1 - Suspected engine room fire and passenger evacuation onboard Fitzroy Flyer and Case Study 2 - Crew member fatality on board Toucan Arrow.
You have been asked to assist with applying required incident investigation processes which have several key components.
Assessment instructions
For each case study write a communication to other advisors who are also participating in the workplace investigations. The two communications may be formatted as a report, briefing paper or other suitable workplace communication, using correct format, spelling, grammar and industry-specific terminology.
You will find templates for workplace documents at virtual enterprise Plan or from another suitable organisation. Your response to be approximately 400 to 450 words.
In the two documents you will apply root cause analysis techniques and include the following:
a. A definition of the incident (what happened).
b. An explanation of the impact of the incident.
c. Identification of at least four possible causal factors.
d. Interpretation of root cause/s - what is the actual reason the incident occurred.
e. At least two draft recommendations for the WHS incident investigation team to consider (based on your root cause analysis and your research in Part 2).
You may submit the two communications as an Appendix to this document, or as separate documents.
