Case: Concorde Air France Flight 4590 crash

Material Science and Engineering CHEG 2013

Aim: To illustrate the importance of materials science in a real life situation. However, technology was not the only contributor to these disasters, other issues such as management, ethics, political and social pressures all had their part to play. Examine the main issues surrounding the incident and demonstrates the complex nature of a real-life problem.

Key skills: Group study, oral communication, research skills, time management

Case 1: (Group 1) Florida International University pedestrian bridge collapse 2018. Give your investigation results based on fracture mechanics, reliability and safety.

Case 2: (Group 2) On April 1 2011 a Southwest Airlines jet tore open during flight. It has raised questions about the reliability of industry procedures used to test airplanes. Give your investigation results based on fracture mechanics, reliability and safety.

Case 3: (Group 3) Hyatt Regency Hotel Walkway Collapse (1981) - On July 17, 1981, two vertically walkways collapsed at the Hyatt Regency in Kansas City, falling into the hotel's lobby. 114 people were killed at what was then the deadliest structure collapse in the history of the United States. Serious flaws were found in the design of the walkways. The engineers who had approved the final drawings of the walkways were convicted of gross negligence and misconduct and lost their engineering licenses

Case 4: (Group 4) The Space Shuttle Challenger 51-L was the 25th mission in NASA's STS program. On Jan. 28, 1986, STS 51-L exploded shortly after liftoff, destroying the vehicle and all of its seven crew members.

Case 5: (Group 5) Sinking of the Titanic. The sinking of the Titanic is history's most epic sea disaster and the topic of endless books, feature films and documentaries.

Case 6: (Group 6) The Columbia Space Shuttle Disaster. On February 1, 2003, the Space Shuttle Columbia disintegrated upon reentering Earth's atmosphere, killing all seven crew members. The disaster was the second fatal accident in the Space Shuttle program after Space Shuttle Challenger, which broke apart and killed the seven-member crew 73 seconds after liftoff in 1986.

Case 9: (Group 9) United Flight 232: The engine failure on United Flight 232, which led to over 100 deaths in 1989, was the result poor maintenance on the part of United Airlines Maintenance that resulted from cost-cutting efforts.
