Mrs. Geraldine Francis is a 68 year old female patient of Dr. Smith's. She is 36 hours post of total left hip replacement following a fall at home. She had an uneventful night except for some discomfort in her hip and lower leg. At that time, she received 2 Percocet tabs. Her pain was at a level 8. She stated relief by 0300 and was sleeping when checked at 0600. She is voiding in good amounts amber urine per bed side commode. She is taking po fluids well. Saline lock intact to left arm. Toes are warm, pink. With pedal pulses 2+ bilaterally. She transfers well with a walker. She does have a history of atrial fibrillation and a CABG 3 years ago. She has NTG available prn but has had no complaints of chest pain during his hospitalization. She is scheduled for discharge tomorrow AM if he does well in PT today. She needs much encouragement as her activity level has not been progressing as well as PT would like her to.
At 9:30 AM the patient puts on his call light. You enter and she is complaining of pain in her leg.

Brad is a 63 year old male with Type 1 Diabetes Mellitus. He is admitted to the med-surg unit with severe pain to the R) foot and lower leg, they are cool to touch with absent pulses (per Doppler). Arteriogram shows severe atherosclerosis of the R) Popliteal artery with complete obstruction of blood flow. Alteplase was administered with no success. The R) foot and leg become necrotic. He is to receive an above-the-knee amputation (AKA). He undergoes the surgery with no complications and returns to you.
