Assignment: Evidence-Based Measures for Evaluating Healthcare Improvements
Note: You must also have completed and passed the CPE for this course before beginning this performance assessment.
All specialty courses in the Leadership and Management program will have a summative assessment that consists of an authentic performance assessment. Together, the courses scaffold the tasks of your healthcare improvement project (HIP) through the project management phases: project initiation, planning, implementation, and evaluation. Each performance assessment will focus on these phases by requiring a real-world approach to improving healthcare with the implementation of a healthcare improvement project. Finally, each phase will be described in the different sections of your HIP paper. Evidence-based measures for evaluating healthcare improvements are essential components of the planning phase of a HIP. Another important component is learning to identify key performance indicators and metrics used to determine the success of a HIP.
In this performance assessment, you will describe the collaborative process used to identify the data elements needed for project reports and for determining the success of the implementation project. You will also describe the collaborative process used to create the project data management plan. You will have the opportunity to develop a project results dissemination plan, which will allow you to effectively share your project findings, as well as the opportunity to develop a project closure plan, which will allow you to properly conclude your project.
This task requires the submission of the provided "D159 HIP Paper Template", including the following sections of your HIP paper, which you will be developing in this performance assessment:
1) "Project Reports" ;
2) "Data Management Plan" ;
3) "Results Dissemination Plan" ;
4) "Project Closure Plan"
The following template should be completed and submitted as an appendix to your provided HIP paper template:
• "SMART Goal" (completed in D156)
During your clinical practice experience (CPE), you will be evaluated on whether the "Project Reports" and "Data Management Plan" elements are completed. In this task, these elements will be evaluated on content and quality of completion. Your CPE will provide you with an opportunity to practice and improve these items before including them in your performance assessment.
Project Reports
I. Identify three data elements that your team determined would be necessary for evaluating the success of your proposed HIP.
A. Describe why each of the three data elements from part A are essential for evaluating the success of the HIP.
Data Management Plan
II. Describe the source(s) of each of the three data elements from part A that will be used to measure the success of the HIP.
III. Identify two process KPIs.
Note: You may present your process KPIs from D156, if applicable.
IV. Describe one internal or external benchmark established by your organization to measure the SMART goal against.
V. Evaluate the process you will use to collect your data by doing the following:
A. Explain how the project team will collect the data (e.g., downloading data from a system, gathering data from a chart audit or survey).
B. Explain how the project team will protect the data.
C. Identify the parameters you will use to collect data (e.g., dates, data elements, calculated fields).
VI. Evaluate the process you will use to analyze and interpret your data by doing the following:
A. Describe a method you will use to analyze the three essential data elements from part A (e.g., descriptive statistics, such as counts, averages, or percentages).
B. Describe the process for how you will interpret initial results.
C. Predict and analyze an issue that may affect your proposed project results.

VIII. Create your plan for the stakeholder wrap-up session by doing the following:
A. Describe the method you would use to acknowledge the organization for its time and support.

C. Describe a plan for sustaining the project.
IX. Acknowledge sources, using in-text citations and references, for content that is quoted, paraphrased, or summarized.
X. Demonstrate professional communication in the content and presentation of your submission.
